Classifications:

Make game fee check payable to Shoot Out Hockey and make U.S. Money Order payable to Referees. Credit card payments can only be accepted for the game fee.  Referee payment can only be accepted with a U.S. Money Order .

• As a condition of acceptance, "Out-of Town" teams must stay at an authorized Big Bear hotel.
Release and waiver - In consideration of my being permitted to compete in, travel with, dine with, officiate in, observe, work for, or in any other manner participate in a Shoot Out Hockey Inc. / Big Bear Tournament, and on behalf of myself, my family, travel companion(s), personal representative(s), and assignees, I hereby release and discharge the promoters, all other participants, all officials, sponsors, advertisers, and the owners, operators, and/or lessees of any equipment, facilities, or premises used to conduct the event (including without limitation Shoot Out Hockey, Inc., and its employees, and also including all third parties providing transportation, meals, lodging, accommodations, and other facilities and services) and each of them and any of their agents or employees ( hereinafter called the release) from all liability to me, my family, travel companion(s), heirs, personal representative(s), and assigns, for any and all loss or damage, and any claim or demand therefore due to the delay or cancellation of tournament games, activity, transportation, accommodation, or event, or to any property damage, personal injury, or other act of God, or otherwise, during my participation in the event, and I hereby agree that the courts of the state of Michigan shall be the sole choice of forum (and the laws of the state of Michigan shall govern and serve as the choice of law) for purposes of the resolution of any dispute arising from my competition in, travel with, dining with, officiating in, observing, working for, or in any other manner participating in the Shoot Out Hockey, Inc. / Big Bear Tournament, and I hereby grant to Shoot out Hockey, Inc. the irrevocable right to use my name, face or other likeness for the purpose of advertising this event or similar events.  Must be 18 years of age to sign or attach a letter from parent/guardian signing release and waiver on behalf of minor.  Signatory parties acknowledge receipt of the general information packet pertaining to the above mentioned tournament and containing a copy of the above release and waiver provisions ( including choice of aforementioned choice of law provisions), acknowledge their agreement to be bound by such release and waiver provisions, and agree to abide by the terms and conditions of such general information packet.
